Théophano ou Theophanu ou Theofano ou encore Theophania (en grec Θεοφανώ Σκλήραινα / Theophanô Sklèraina) (v. 960 † 991) est une princesse byzantine de la Dynastie macédonienne, donnée en mariage à Otton II du Saint-Empire, et devenue impératrice du Saint-Empire. L'une des souveraines les plus influentes de l'Europe du Moyen Âge, elle régna onze ans aux côtés d'Otton II et, à la mort de celui-ci en 983, sept ans, seule, comme régente de son fils Otton III.
